随着定量组织血氧测量技术的发展,利用近红外光无创精准识别脑组织与骨骼肌微血管功能障碍,正逐渐成为未来临床诊疗中的重要方向。时域近红外光谱技术(Time-Domain Near Infrared Spectroscopy,TD-NIRS)是一种先进的生物光子学方法,能够以极高的精度和可重复性对组织内的血氧水平进行定量测量,该技术已在先进临床研究中得到应用。在本次讲座中,Michele Lacerenza 博士将介绍最新一代 TD-NIRS 设备在临床与实验室研究中的应用进展。基于其与多家世界顶尖重症监护病房(ICU/NICU)及高校的合作,他将展示一系列“从实验室到临床”的转化研究方案,包括:(1)重症脑血流动力学监测及外周肌肉微血管反应性测试;(2)微血管功能障碍的分型与纵向评估指标的建立;(3)通过改进浅层信号抑制,实现更准确的脑功能激活测量;(4)基于肌肉氧合动力学与恢复过程的运动生理学研究。讲座将重点讨论设备的可用性、质量控制及数据报告规范等关键问题,这对于推动多中心、高影响力的 TD-NIRS 临床研究具有重要意义。此次活动也将为在上海地区启动相关合作研究、开展试点项目提供契机,促进医学诊断与生物医学研究的进一步融合与发展。
The use of quantitative tissue oximetry devices, to accurately and non-invasively identify microvascular impairment in cerebral tissue and skeletal muscle, is expected to play a pivotal role in future clinical practice. Time domain near infrared spectroscopy (TD-NIRS) is an advanced optical technique that facilitates precise, reproducible, and quantitative tissue oxygenation measurements and it is currently employed in cutting-edge clinical research. Dr. Michele Lacerenza will present the latest advancement of TD-NIRS devices for clinical research and laboratory applications. Drawing on collaborations with leading ICUs/NICUs and universities worldwide, Lacerenza will showcase translational protocols for: (1) critical care cerebral hemodynamic monitoring and microvascular reactivity testing on peripheral muscles; (2) microvascular impairment phenotyping and longitudinal endpoints; (3) brain functional activation with improved superficial-layer rejection; and (4) exercise physiology through muscle oxygenation kinetics and recovery dynamics. Emphasis will be placed on device usability, quality assurance, and reporting standards to accelerate high-impact, multicenter TD-NIRS studies, including opportunities to initiate pilot projects and new collaboration in Shanghai fostering advancement in medical diagnosis and biomedical research.

Michele Lacerenza 博士现任意大利 PIONIRS 公司的首席技术官(CTO)兼联合创始人。他于 2018 年在米兰理工大学(Politecnico di Milano)获得物理工程硕士学位(研究方向为纳米光学与光子学),并于 2022 年获得物理学博士学位,期间的研究重点是面向脑和肌肉血氧成像的高集成光学系统的开发。Lacerenza 博士的工作涵盖时域近红外光谱技术(TD-NIRS)的仪器系统设计、定量组织血氧测量方法,以及从神经生理学到临床监测的多领域转化应用。他共同创立的 PIONIRS 公司是米兰理工大学正式认可的衍生企业,并得到了 Micro Photon Devices公司的大力支持。Micro Photon Devices公司位于意大利博尔扎诺(Bolzano),在单光子探测技术领域具有多年的深厚积累。依托这一坚实的技术背景,PIONIRS 致力于研发并推广新一代 TD-NIRS 设备,推动其在全球范围的临床与科研应用。
Michele Lacerenza is CTO and co-founder of PIONIRS s.r.l. He received an M.Sc. in Physics Engineering (nano-optics and photonics, 2018) and a Ph.D. in Physics (2022) from Politecnico di Milano, where he focused on compact photonic platforms for cerebral and muscle oximetry. His work spans instrumentation design for time-domain NIRS (TD-NIRS), quantitative oximetry methods, and translational applications from neurophysiology to clinical monitoring. Officially acknowledged as spin-off company of Politecnico di Milano and also supported by Micro Photon Devices S.r.l., a company based in Bolzano (Italy) having years of experience in single-photon detection, PIONIRS is committed to deliver to the world the next-generation devices for TD-NIRS.
